PRIVACY CONSENT

I give consent for From Scratch Farm to obtain and use all individual information for the purposes of providing care, financial transactions or direct mailings. Information will be retained for 5 years and then discarded.

 

ROUTINE AND EMERGENCY PICK-UP: I acknowledge that only authorized individuals pick up the Participants and that these individuals are available to pick up the Participant both at the end of program or during programs in the event of an emergency or for other reasons. Any authorized individual picking up the Participant must bring with them Government Issued Identification in order to verify their identity. All authorized people understand that they may be called upon to pick up the Participant either at the end of their workshop/activity or during the session, in case the primary caregiver is not available.

 

Emergencies: I acknowledge that in the event of an emergency, the guardian/primary caregiver listed and/or the individuals listed in the application will be contacted and are authorized to pick up the Participant. I acknowledge that I must ensure that all emergency contacts are aware of this responsibility prior to the Participant’s arrival at the Farm. 

  

INJURY/ILLNESS: I acknowledge that From Scratch Farm is not responsible for any illness or accidental harm that might occur to the Applicant. The term illness includes but is not limited to any infectious diseases or other illnesses that might occur in the community. Accidental harm includes but is not limited to harm arising from any event that was not intentionally or willfully caused by any member of From Scratch Farm  staff or volunteer. For example, if the Participant was to be injured as a result of a trip and fall incident, if they were to be hit in the course of play, or if they were injured by another participant, etc.
 

COVID-19: Prior to entry at From Scratch Farm, all participants will be screened for symptoms and exposure to COVID-19. Participants may be denied entry to Farm property, and reservation canceled if the screening is not passed. 
 

I understand that the Participant must be vaccinated and immunized against COVID-19 to attend sessions.

In the event of a suspected/confirmed case of COVID-19, the participant will be isolated, and ride providers will be contacted to pick the participant up immediately.
 

 

APPLICATION CONSIDERATION:  I acknowledge that I understand that From Scratch reserves the right to deny an application for any reason.  Common reasons for denying an application: communication needs are too great for staff to manage, behavioural needs are too great for staff to manage, participant cannot self-transfer to toilet, or medical needs are too great for staff to manage.

For the safety of those participating, individuals with behavioural challenges who pose a risk of danger to themselves or others, and who require a Behavioural Support Plan and facilitator, are not able to participate in our programming. It is our hope in the future to offer some programming that could be more inclusive.

If deemed appropriate, we may ask if you are interested in the “bring a buddy” program, in cases where a little extra help is required for challenges that can be supported by having 1:1 support and that do not jeopardize the experiences of the other Participants. 

At this time the From Scratch Farm is unable to provide 1:1 staffing for those with greater physical or behavioral challenges in our sessions.  In the case where an individual could participate more fully with the help of a 1:1 aide for their physical challenges, we welcome those participants to “bring a buddy” (staff or family member) to assist them.
A scalable fee for service will be provided if staffing is brought for the individual.

We recognize that everyone has days where they require extra support, but if a group activity is disrupted regularly by the same individual repeatedly, a request will be made for them to “bring a buddy”, or they will be asked to cease attending activities. 
 

 

DISCLOSURE:  I acknowledge that I understand that failure to include any information about properly caring for the Applicant in this application or in writing prior to the Applicant’s participation in sessions will result in the Applicant being removed from the program and asked to leave the session. I have disclosed all the information above to any individual that may be involved in the application process, pick-up/drop-off, or communication with From Scratch Farm regarding the applicant for the 2023 season.
